You must be at least 18 years old and legally capable of entering into an agreement to book services for yourself. A parent, legal guardian, managing conservator, or other legally authorized representative must book and consent for a minor or an individual who cannot legally consent, subject to applicable law and the
requirements of the testing laboratory.
Submitting a booking request does not guarantee an appointment. A booking becomes confirmed only after HomeAccess Labs issues a confirmation and any required payment or deposit is received.
HomeAccess Labs provides only the service identified in the booking confirmation and applicable consent documents. Unless expressly
stated in writing and performed by an appropriately licensed clinician, our services do not include diagnosis, medical treatment,
prescribing, emergency care, or interpretation of laboratory results.
Specimen collection does not guarantee that a laboratory will accept, test, or produce a result from a specimen. Laboratories may reject or require recollection of a specimen because of insufficient
quantity, labeling issues, temperature, integrity, chain-of-custody requirements, transportation conditions, or other laboratory criteria.
If you believe you are experiencing a medical emergency, call 911 or seek immediate emergency care. Do not use the website or a HomeAccess Labs appointment as a substitute for emergency services.
You agree to provide accurate and complete contact, identification, appointment-location, service-selection, participant, ordering-provider,
employer, attorney, and billing information. You must promptly notify us of changes that may affect the appointment.
Certain services require additional information before confirmation, including a valid laboratory requisition or provider order, employer
authorization, testing reason, donor information, identity documents, photographs, legal-authority documents, or information concerning
other DNA-test participants.
You are responsible for providing a safe, lawful, reasonably private, sanitary, and accessible location for the appointment. You must disclose known hazards, access restrictions, parking requirements, security procedures, animals, communicable-disease concerns, or other conditions that could affect safe performance of the service.
HomeAccess Labs may decline, pause, relocate, or end an appointment when conditions are unsafe, unlawful, threatening, unsanitary, lacking reasonable privacy, or incompatible with specimen integrity or collection requirements. Fees may apply when an appointment cannot be completed because the customer, donor, participant, location, or required documentation is not ready.
All required participants must be present on time with acceptable, unexpired identification and any required orders, authorizations, employer documents, or legal paperwork. You are responsible for following preparation instructions, including hydration, fasting, medication disclosures, identification requirements, or restrictions
communicated for the selected service.
HomeAccess Labs may refuse collection when identity cannot be verified, valid consent cannot be obtained, required documentation is missing, a participant appears unable to consent, or collection would violate applicable law, professional standards, laboratory instructions, or chain-of-custody procedures.
Prices displayed on the website are subject to confirmation and may vary based on the selected service, number of participants or donors, appointment location, mileage, after-hours service, laboratory charges, expedited processing, recollection, legal chain of custody, or employer-specific requirements.
You authorize HomeAccess Labs and its payment processor to charge the disclosed amount for the services you request. You remain
responsible for amounts not paid by an employer, attorney, insurer, or other third party unless HomeAccess Labs agrees otherwise in writing.
Cancellation and rescheduling terms: Cancellations are refundable within at least 24 hours prior to the appointment.
A customer or participant who is absent, more than 15 minutes late, unprepared, lacks identification, refuses required collection procedures, or cannot provide valid consent may be treated as a late cancellation or no-show.
If HomeAccess Labs must cancel or reschedule, our responsibility will ordinarily be limited to rescheduling the appointment or refunding amounts paid directly to HomeAccess Labs for the unperformed service, except where applicable law requires otherwise.
Website acceptance of these Terms is not consent to a blood draw, specimen collection, wellness service, DNA test, or drug test. Before performing a service, HomeAccess Labs will require the General Client Consent and the applicable service-specific consent, together with any legally required privacy authorization, guardian authorization, laboratory form, or chain-of-custody document.
You may ask questions before signing a service consent. A collection or wellness service will not begin until required consent and documentation are completed. Refusal to sign or withdrawal of consent may prevent completion of the service and may affect refund eligibility under the disclosed cancellation policy.
HomeAccess Labs may coordinate with independent laboratories, medical-review officers, ordering providers, employers, attorneys, couriers, payment processors, or other service providers. Unless expressly identified as our employee or agent, an independent third party is responsible for its own professional services, testing methods, accreditation, result reporting, turnaround time, privacy obligations, and errors.
Estimated laboratory turnaround times are not guarantees. Delays can result from shipping, specimen quality, confirmation testing, medical review, holidays, laboratory workload, legal requirements, or circumstances outside HomeAccess Labs’ reasonable control.
Results will be delivered only through methods and to recipients permitted by law, laboratory rules, the applicable service documents, and a valid authorization when required. You are responsible for providing accurate recipient information and safeguarding any result delivered to you.
A website booking, payment, privacy-notice acknowledgment, or general consent does not by itself authorize disclosure of health, drug-testing, or genetic information to an employer, attorney, family member, insurer, government agency, or other third party.
A valid requisition or provider order may be required depending on the test and laboratory. Collection is subject to the laboratory’s kit, labeling, handling, transport, and acceptance requirements. A recollection may be required if the specimen is rejected, compromised, insufficient, or otherwise unsuitable.
The specific wellness activities will be identified before the appointment. Screening measurements and educational information are not a diagnosis and do not replace evaluation by the customer’s physician or other qualified healthcare professional unless an appropriately licensed clinician expressly provides such services.
The customer must identify whether testing is informational/peace-of-mind or legal chain-of-custody testing. Informational testing may not be admissible in court or accepted by a government agency. Legal testing requires specified identification, witnessed collection, photographs, documentation, laboratory accreditation, and chain-of-custody procedures. Each adult participant and each minor’s legally authorized representative must complete required consent documents.
The booking must accurately identify whether testing is DOT-regulated or non-DOT. DOT collections are governed by applicable federal procedures and the federal Custody and Control Form; HomeAccess Labs will not require a consent, liability waiver, release, or indemnification agreement covering the federally regulated testing process. Non-DOT testing is subject to the requesting organization’s lawful policy, laboratory procedures, and any authorization required to release results.
Our collection, use, storage, and disclosure of personal, health, drug-testing, and genetic information are governed by applicable law and the HomeAccess Labs Website Privacy Policy and Notice of Privacy Practices, as applicable. Those documents are incorporated by reference for website and booking purposes but do not replace a specific authorization when one is legally required.

You consent to receive transactional communications reasonably necessary to schedule and perform requested services, including confirmations, preparation instructions, arrival notices, rescheduling notices, receipts, and requests to complete required forms. HomeAccess Labs is not responsible for delay or failure caused by events beyond its reasonable control, including severe weather, unsafe travel, vehicle failure, public-health events, courier interruption, laboratory closure, supply shortages, power or internet outages, governmental action, or other comparable events. We will make reasonable efforts to communicate and reschedule affected appointments.
We may update website content, service availability, prices, policies, or these Terms prospectively. The version presented when you book generally governs that booking, except when a change is required by law, safety standards, laboratory requirements, or a regulatory authority. Material changes will be reflected by an updated effective date.
These Terms are governed by the laws of the State of Texas, without regard to conflict-of-law principles.
